Insurance and fees, plan by plan.
We're out of network with most insurance companies, but there are several ways to use your benefits. Tell us how you'd pay, and we'll show you what's covered, what to do before your first session, and what you'll pay.

Lyra Health Through your employer
Yes, we work with Lyra Health.
Lyra is a benefit some employers offer, and it often covers a set number of sessions in full. We bill Lyra directly, so you won't get invoices from us.
Before your first session
- Ask your HR or benefits team whether you're eligible, how many sessions you have, and whether your plan covers psychiatry as well as therapy.
- Check how many sessions you have left this year in your Lyra profile, or ask Lyra's care team at care@lyrahealth.com or (877) 505-7147. Sessions with other providers count toward the same total.
- When you book, tell us your company's name. Booking for a family member? We'll also need their name, date of birth and how they're related to you. If your employer gave you a Lyra code, have it ready too.
- Add a card when you book. It's only charged if Lyra doesn't cover a late cancellation or a missed session.
What you'll pay
- Many plans cover your sessions in full. Some have a copay or cost share through your health plan; Lyra's care team can tell you yours.
- Every Lyra plan covers therapy, but only some employers cover psychiatry. If yours doesn't, you can still see our psychiatrist at our usual fee, with a superbill for your insurer.
- Used all your sessions? Some employers let you keep going with Lyra through your health plan. Add your plan in your Lyra profile (Profile, then Health Plan), with the member ID exactly as it is on your card, and a payment card. If yours doesn't, you can carry on with us using out-of-network benefits or paying yourself.
Good to know
- Lyra covers standard sessions of 45 to 50 minutes, usually once a week. Longer sessions aren't covered.
- Cancel or reschedule at least 48 hours ahead. If Lyra doesn't cover a late cancellation or missed session, the session fee is yours.
- Turning 18? You'll need your own Lyra profile, and Lyra's care team can help you set it up.
- Your employer doesn't see your records or what you talk about.
Spring Health Through your employer
Yes, we work with Spring Health.
Spring Health is a benefit some employers offer, much like Lyra. Spring Health matches you with one of our clinicians and pays us directly, so you won't need a card on file with us.
Before your first session
- Check with your HR or benefits team that you have Spring Health, and how many sessions it covers.
- Create your Spring Health account.
- Email Spring Health's care team at careteam@springhealth.com and ask to be matched with one of our clinicians. If you have someone in mind, give their name.
- Once you're matched, you can book your first session.
Good to know
- Your appointments and forms are in Spring Health's own system, rather than our Client Portal.
- Questions about your sessions or any cost go to Spring Health, as they take care of billing.
- Your employer doesn't see your records or what you talk about.
Modern Health Through your employer
Yes, we work with Modern Health.
Modern Health is a benefit some employers offer. Modern Health refers you to one of our clinicians and takes care of billing, so you won't get a bill from us.
Before your first session
- Check with your HR or benefits team that you have Modern Health, and how many sessions it covers.
- Set up your Modern Health account, if you haven't already.
- Ask Modern Health to connect you with one of our clinicians.
- Once they've referred you, we'll email you to set up your first session and send our intake forms. There's no card to add.
What you'll pay
- Any copay is set by Modern Health, so check your Modern Health account. We don't see it.
- Seeing our psychiatrist through Modern Health? Pay your copay in the Modern Health app before the visit. If it isn't paid in time, the visit is cancelled automatically.
Good to know
- Need to cancel, or missed a session? We won't charge you. Modern Health can tell you how they handle it.
- Leaving your job? The benefit usually ends when you leave, though some employers allow a grace period. Ask your HR team, including whether it can continue under COBRA.
- Modern Health's member support is at help@modernhealth.com.
Kaiser Referral needed
Yes, with an external referral from Kaiser.
We see Kaiser members for therapy and psychological testing once Kaiser has referred you to us, and we bill Kaiser for you.
Before your first session
- Contact your Kaiser clinic and complete their screening.
- Ask them for an external referral to Mind & Body Garden. Once it's approved, Kaiser gives you an authorization for your care with us.
- Book with us once you have it. Without a valid referral and an active authorization, Kaiser won't reimburse your sessions.
- In your intake forms, add photos of the front and back of your Kaiser card, the policyholder's name and date of birth, and a card on file.
What you'll pay
- We bill Kaiser directly. Any copay, or anything that goes toward your deductible, is charged to your card only after Kaiser has processed the claim, and we let you know before we do.
- If you haven't met your deductible yet, Kaiser may apply your sessions to it, so you'd pay toward it first.
- For testing, that charge usually comes about a month after the assessment.
- Questions about what Kaiser paid? Kaiser Member Services can explain.
Good to know
- Before each session, you'll get a short Kaiser questionnaire. Please fill it in: it helps Kaiser follow your care with us.
- Your authorization lasts for a set time. If you're in ongoing therapy, Kaiser usually renews it and sends you a letter before it ends. No letter a week before it expires? Call Kaiser and ask for a renewal.
- Have an older referral? Call your clinic to check it's still valid.
- If you have other insurance as well and Kaiser isn't your primary plan, let us know.
- For testing, Kaiser may refer members for evaluations such as autism and ADHD. For others, ask Kaiser whether they'll cover it and request pre-authorization; if they won't, our self-pay rates apply.
Through Headway
Yes, with some of our clinicians.
They take through Headway, a service that checks your coverage and bills your insurer for you. You pay only your share, such as a copay.
Before your first session
- Choose one of the clinicians above. With our other clinicians, you can still use out-of-network benefits.
- Send us photos of the front and back of your insurance card, and your date of birth. We'll set you up with Headway and in our Client Portal.
- If you like, create your own Headway account at care.headway.co to add your insurance and see your copay before you start.
- Fill in our intake forms. There's no card to add, since Headway takes care of billing.
What you'll pay
- Your share depends on your plan, and Headway shows it to you. It can differ from one clinician to another.
- Questions about a bill go to Headway, which works with your insurer directly.
Good to know
- A letter from your insurer saying you owe more? It's usually a processing delay. Send it to us and we'll sort it out with Headway.
- At the start of each year, or if your plan changes, send us your current card so your coverage can be checked again.
- Your video link may come from Headway or from our Client Portal, depending on your clinician.
Another insurance plan Out of network
You can use your out-of-network benefits.
We're out of network with most insurance companies. You pay our fee, and each month we send you a superbill to claim part of it back. Many plans repay 50 to 80 percent once your deductible is met.
Before your first session
- Call your insurer, using the number on your card, and ask about out-of-network benefits for outpatient mental health care. Our questions can help.
- Having psychological testing? Ask your insurer to pre-approve it first.
- Add a card when you book. Each session is charged to it.
- As you'll be paying us directly, you'll also receive a Good Faith Estimate before you start.
What you'll pay
- Our fee at each session: $200 to $300 for therapy, depending on the clinician, or $500 for a first psychiatry evaluation and $300 for each follow-up.
- Then your insurer pays you back their share. Plans typically repay 50 to 80 percent once your deductible is met, but only they can confirm your rate.
Good to know
- Some of our clinicians take plans such as Aetna and Blue Cross through Headway. If your plan is in the list above, choose it to see who.
- Coaching isn't covered by insurance and is self-pay only.
- We're not able to call your insurer for you, but we're happy to tell you what to ask.
Paying yourself Self-pay
$200 to $300 per therapy session, depending on the clinician.
Each clinician's rate is on their profile. Psychiatry and psychological testing have their own fees.
per therapy session, depending on the clinician
a first psychiatry evaluation
each psychiatry follow-up
Before your first session
- Check your clinician's rate on their profile on Our Team.
- Add a card when you book. Each session is charged to it, and FSA and HSA cards work too.
- Before you start, you'll receive a Good Faith Estimate, as the No Surprises Act requires. It estimates weekly sessions to the end of the year, so the total can look large, but you're only charged for the sessions you attend.
Good to know
- Thinking of claiming from your insurance later? Your superbills are in the Client Portal each month.
- Some of our clinicians keep a few reduced-fee places. Ask us who has openings.
- Cancel or reschedule 48 hours ahead at no charge. Later cancellations and missed sessions are charged the full session fee.
FSA or HSA Accepted
Yes, FSA and HSA cards are accepted.
Use them like any card, through our secure Client Portal, with any of our clinicians.
per therapy session, depending on the clinician
a first psychiatry evaluation
each psychiatry follow-up
Before your first session
- Add your FSA or HSA card when you book, or later in the Client Portal.
- You'll receive a Good Faith Estimate before you start, as you're paying for yourself.
Good to know
- If your plan asks for receipts, download your statements or superbills from the Client Portal.
- Cancel or reschedule 48 hours ahead at no charge.
Not sure yet We'll help
That's okay. Most people aren't sure at first.
We'll help you work it out before your first session. Here's where to start:
- Ask your HR or benefits team whether you have Lyra Health, Spring Health or Modern Health. These often cover a set number of sessions in full.
- Look at your insurance card. With Kaiser, you'll need a referral first. For other plans, check the list above: some of our clinicians take plans such as Aetna and Blue Cross through Headway.
- Call your insurer and ask about out-of-network benefits. Our questions can help.
- Or just ask us. Our care team will go through it with you, in English or Chinese.
Before you use your insurance with us
A short call now can save surprises later. We're not able to call your insurer or HR team for you, but here's what to ask, and what to have ready when you book.
Ask your insurer
- Do I have out-of-network benefits for outpatient mental health care?
- What's my out-of-network deductible, and how much of it have I met this year?
- Once it's met, what percentage do you pay back, and is that a share of the full fee or of an amount you set?
- Is there a limit on sessions each year, and do I need approval first?
- Are online sessions covered the same way as in person?
- How do I send you a superbill, and how soon will I be paid back?
Ask your HR or benefits team
- Do we have Lyra Health, Spring Health or Modern Health?
- How many sessions does it cover each year, and does that include family members?
- Does it cover psychiatry as well as therapy?
- Once the sessions are used, can I keep going through my health plan?
- What happens to the benefit if I leave the company?
Have these ready when you book
- Photos of the front and back of your insurance card
- The policyholder's name and date of birth
- Your employer's name, if you're using Lyra Health, Spring Health or Modern Health
- Your Kaiser referral, if you're a Kaiser member
- A credit, debit, FSA or HSA card for the Client Portal, unless Headway, Spring Health or Modern Health takes care of your billing
If your insurer asks for service codes: 90791 for a first therapy session, and 90834 or 90837 for ongoing sessions. For psychiatry, 90792 for the first evaluation, and 99213 or 99214 for follow-ups.
Claiming back with a superbill
If you see us out of network, a superbill is how you get part of your fee back from your insurer. It takes three steps.
- You pay at each sessionYour session fee is charged to the card in your Client Portal.
- Your superbill arrivesEach month we send you a superbill, which is also saved in your Client Portal: a detailed receipt of your sessions, with the diagnosis and service codes your insurer needs.
- Your insurer pays you backSend it to your insurer through their claims process. Plans typically repay 50 to 80 percent once your deductible is met.
- Want several months on one superbill? Ask us and we'll put them together.
- Is a superbill missing its diagnosis code? Let us know, as your insurer needs it.
- Having psychological testing? A superbill is available on request.
Psychiatry and psychological testing
Both have their own fees, and insurance works a little differently for each.
Psychiatry
- First evaluation
- $500
- Each follow-up
- $300
- With Lyra Health, only some employers cover psychiatry, so check with your HR team first.
- With Modern Health, pay your copay in the Modern Health app before each visit, or it's cancelled automatically.
- With other plans, you pay our fee and claim part of it back out of network with your superbill.
- Our psychiatrist is licensed in California, so you'll need to be in California for your visits.
Psychological testing
- IQ test
- $850
- Autism evaluation
- $3,000
- ADHD evaluation
- $4,500–5,500
- Autism and ADHD together
- Up to $5,500
- Test accommodations
- $3,000–5,500
- Fertility evaluation
- From $800
- We confirm your fee before you start, and your card is charged once the assessment is complete.
- Testing is usually self-pay. Many plans cover it with pre-approval, so ask your insurer to approve it first. Evaluations for test accommodations and fertility treatment usually aren't covered.
- Kaiser may refer members for some evaluations, such as autism and ADHD.
- A superbill is available on request.
Good to know
Something else on your mind? Our FAQ covers much more, and our care team is always happy to talk it through.
How do I pay?
By credit or debit card, including FSA and HSA cards, through our secure Client Portal. You'll add a card when you book, and session fees are charged to it. For psychological testing, the card is charged once the assessment is complete. If Headway, Modern Health or Spring Health takes care of your billing, you won't need a card on file with us.
What if I need to cancel or reschedule?
Let us or your clinician know at least 48 hours before your appointment and there's no charge; you can also do it in the Client Portal. Cancellations with less notice, and missed sessions, are charged the full session fee. With Lyra, the fee is yours only if Lyra doesn't cover it. With Modern Health, we never charge you, and Modern Health can tell you how they handle it.
What is a Good Faith Estimate?
If you don't have insurance, or aren't using it, the No Surprises Act gives you the right to a Good Faith Estimate: a written estimate of what your care is expected to cost. You'll receive yours before your first session. For therapy, it covers weekly sessions to the end of the year, so the total can look large, but you're only charged for the sessions you attend. If a bill comes to at least $400 more than your estimate, you can dispute it, so keep a copy. Our Good Faith Estimate page shows what yours will look like and explains your rights.
Do you offer reduced fees?
Some of our clinicians keep a few reduced-fee places. Ask our care team, and we'll tell you who has openings and what's possible.
My insurance or job has changed. What should I do?
Let us know as soon as you can, and send us your new card so we can update your details. If your sessions come through Lyra, Spring Health or Modern Health, check with your HR team how long the benefit lasts after you leave. You can always carry on with us using out-of-network benefits or paying yourself.
Will my insurer or employer know what we talk about?
No. If you use insurance, we share only what's needed to bill it, such as session dates and a diagnosis code, never what you say in sessions. Your employer doesn't see your records, including when you come through a program like Lyra.
Is coaching covered by insurance?
No, coaching is self-pay only. Some of our clinicians offer it by video to people outside the states where they're licensed. It works on the same things as therapy, but it can't include a diagnosis.
Still have a question about cost?
Our care team is happy to go through your options with you, in English or Chinese. Send us a short message, or call or text (650) 434-2563.
